In addition to the documents available to download, this page also includes a range of forms that clubs, team managers and officials may need throughout the 2026–2027 season. Please ensure you use the latest versions of these forms whenever required.
- Dual-Registration Application
This form is to allow clubs to easily apply for Dual registrations as described in the regulations - Player yellow card limit declaration form
In accordance with regulation 12.2.1 (a)- Players will face incremental match suspensions for yellow cards accrued during the season in

In accordance with regulation 12.2.1 (b)- Teams will face incremental penalties for yellow cards accrued during the season in League matches - Post Deadline Transfer Requests
As per the 2026/27 adult league regulations, the deadline for both new registrations and player transfers is 23:59 on 31st January. Following this date any transfer needs to be approved by the ALMC prior to being started on GMS.
